Description

Authorize to enter into a sole source amendment to an existing contract with Southwestern Community Services, Inc., Keene, NH (originally approved by G&C on 12/18/19, Item #32), for the continuation of Rapid Re-Housing, Permanent Housing program and support services for individuals and families facing homelessness through the Federal Continuum of Care Program, by exercising a contract renewal option by increasing the total price limitation by $123,475 from $359,036 to $482,511 and extending the completion date from December 31, 2022 to December 31, 2023. Effective retroactive to January 1, 2023, upon G&C approval. 100% Federal Funds.
