Authorize to enter into a sole source contract amendment with the Disability Rights Center, Concord, NH (originally approved by G&C on 3/9/22, Item #41), by increasing the contract price by $25,000 from $50,000 to $75,000 and extending the completion date from April 15, 2023 to June 30, 2023, for the purpose of building and maintaining a coalition of people with disabilities, family members, advocates, service providers and other to identify barriers and opportunities to expand the use of supported decision making in NH. Effective upon G&C approval. 100% Federal Funds.
