Authorize to enter into a retroactive amendment to an existing contract with OZ Systems USA, LLC, Arlington, TX (originally approved by G&C on 12/2/20, Item #20), for the development, implementation and support of an integrated data management system for infant and newborn hearing screening programs, by exercising a contract renewal option by increasing the price limitation by $336,098 from $339,450 to $675,548 and extending the completion date from November 30, 2022 to November 30, 2025. Effective upon G&C approval. 38.80% Federal Funds, 61.20% Other Funds (Newborn Screening Revolving Fund).
