Description

Authorize to enter into a sole source amendment to an existing contract with ICF Macro, Inc., Fairfax, VA (originally approved by G&C on 12/21/16, Item #22), to continue conducting the annual Behavioral Risk Factor Surveillance System and Asthma Callback Surveys, by increasing the price limitation by $192,512 from $2,759,545 from $2,952,057, and by extending the completion date from December 31, 2022 to December 31, 2023. Effective December 31, 2022, upon G&C approval. 100% Federal Funds.
