Description
Authorize to amend an existing agreement with ICF Macro, Inc., Fairfax, VA (originally approved by G&C on 12/21/16, Item #22), to continue conducting the annual Behavioral Risk Factor Surveillance System and Asthma Callback Surveys, by increasing the price limitation by $148,738 from $2,610,807 to $2,759,545 with no change to the contract completion date of December 31, 2022. Effective upon G&C approval. 98% Federal Funds and 2% Other Funds (University of New Hampshire, Disability and Health Program and Alzheimer’s Association).
