Authorize to enter into a retroactive, sole source amendment to an existing contract with Tri-County Community Action Program, Inc., Berlin, NH (originally approved by G&C on 6/28/23, Item #42, archived document ), for the provision of housing services continuum of care projects, and to modify the scope of services to replace the transitional housing program with a permanent supportive housing program, in accordance with Youth Homelessness Demonstration Program requirements, by increasing the price limitation by $31,699 from $1,656,596 to $1,688,295 with no change to the contract completion date of August 31, 2027. Effective retroactive to April 1, 2025, upon G&C approval. 100% Federal Funds
