Authorize to enter into a retroactive amendment to an existing contract with One Call Government Solutions LLC d/b/a One Call, Jacksonville, FL (informational item D, G&C 5-5-21), to continue providing statewide transportation services to COVID-19 vaccination and testing appointments for individuals who do not receive NH Medicaid, by exercising a contract renewal option to extend the completion date from September 30, 2021 to December 31, 2021, with no change to the price limitation of $400,000. 100% Federal Funds.
