Authorize to amend an existing contract with On-Site Medical Services LLC, Charlestown, NH (informational item #J G&C 4-21-21), to continue to provide COVID-19 vaccinations and boosters to individuals across the state, as requested by the Department, by exercising a renewal option by increasing the price by $16,216,374 from $15,666,570 to $31,882,944 and by extending the completion date from December 31, 2021 to March 31, 2022. 100% Federal Funds.
