Authorize to enter into a sole source contract with Mary Hitchcock Memorial Hospital, Lebanon, NH, for the evaluation of integrated primary care, community behavioral health care and wellness services for young people with serious emotional disturbance, severe mental illness, severe and persistent mental illness, in the amount of $455,688. Effective July 1, 2021 or upon G&C approval, whichever is later, through December 31, 2023, with the option to renew for up to one additional year. 100% Federal Funds.
