Description
Authorize to amend an existing contract with Southwestern Community Services Inc., Keene, NH (originally approved by G&C on 12-18-19, item #32), for the ongoing provision of a Rapid Re-Housing, Permanent Housing program and Supportive Services to individuals and families facing homelessness through the Federal Continuum of Care Program, by exercising a contract renewal option, by increasing the total price by $122,202 from $236,834 to $359,036 and by extending the completion date from December 31, 2021 to December 31, 2022, effective January 1, 2022 or upon G&C approval, whichever is later. 100% Federal Funds.
