Description

Authorize to enter into a cooperative project agreement with University of NH, Durham, NH, to support Aging and Disability Resource Centers and NHCarePath system partners with addressing barriers to access and education relative to COVID-19 vaccinations for individuals engaged in the No Wrong Door System. Effective upon G&C approval through September 30, 2022, with the option to renew for up to 2 additional years. 100% Federal Funds.
