Description

Authorize to amend an existing contract with Harbor Homes Inc., d/b/a Harbor Care, Nashua, NH (originally approved by G&C on 12-2-20, item #23), for case management and support services for individuals living with Human Immunodeficiency Virus, by exercising a renewal option, by increasing the price by $1,572,552 from $625,000 to $2,197,552, and by extending the completion date from September 30, 2021 to September 30, 2023. Effective retroactive to September 29, 2021. 100% Other Funds (Pharmaceutical Rebates).
