Description

Authorize to enter into a grant agreement with Community Health Access Network, Newmarket, NH, in the amount of $360,366 to improve the asthma related services in both medical and community organizations to reduce asthma related illness and deaths. Effective retroactive to September 29, 2021 through August 31, 2024, with the option to renew for up to 3 additional years. 100% Federal Funds.
