Description

Authorize to enter into a retroactive amendment to an existing contract with Qualtrics LLC, Provo, UT (originally approved by G&C on 8-14-19, item #4), to provide software to support the NH Expanded Biomonitoring Program and the Health Professions Data Center, by exercising a renewal option, by increasing the price by $90,000 from $60,000 to $150,000 and by extending the completion date from June 30, 2021 to June 30, 2024. 50% Federal, 50% General Funds.
