Description
Authorize to amend an existing contract with the County of Cheshire, Keene, NH (originally approved by G&C on 9-23-20, item #14), for the provision of Care Management Entity services through the Families and Systems Together Forward program, by exercising a renewal option, by increasing the price by $2,300,000 from $1,200,000 to $3,500,000 and by extending the completion date from June 30, 2021 to June 30, 2023. 100% General Funds.
