Description
Authorize to enter into a contract with NH Coalition for Citizens with Disabilities d/b/a Parent Information Center, Concord, NH, for youth health care transition planning and support for children, ages 14 to 21 years, with special health care needs; their families; and their providers, in the amount of $130,000. Effective July 1, 2021 or upon G&C approval, whichever is later, through June 30, 2023, with the option to renew for up to 2 additional years. 25% Federal, 75% General Funds.
