Authorize to enter into a sole source amendment to an existing contract with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 9-13-17, item #13), to continue to serve as the Administrative Lead Organization of a statewide Health Care Coalition of organizations from across the health care sector to prepare for, respond to, and recover from emergencies, by increasing the price by $615,000 from $3,612,099 to $4,227,099 and by extending the completion date from June 30, 2021 to June 30, 2022. 100% Federal Funds.
