Description
Authorize to enter into a sole source amendment to an existing contract with Tri-County Community Action Program Inc., Berlin, NH (originally approved by G&C on 6-19-19, item #42), to continue to provide services to homeless individuals with serious mental illness through the Project for Assistance in Transition from Homelessness program, by exercising a contract renewal option by increasing the price by $104,744 from $104,744 to $209,488 and by extending the completion date from June 30, 2021 to June 30, 2023. 100% Federal Funds.
