Authorize to amend a sole source contract with NH Coalition for Citizens with Disabilities d/b/a Parent Information Center (originally approved by G&C on 5-1-19, item #67), to continue to provide support for NH’s Pyramid Model implementation, scale-up and sustainability efforts to include new locations across NH, by increasing the price by $387,791.80 from $477,277.16 to $865,018.96, and by amending the completion date from June 30, 2021 to June 30, 2022. 100% Federal Funds.
