Authorize to accept and expend funds from the Centers for Medicare & Medicaid Services, entitled Money Follows the Person Rebalancing Demonstration, in the amount of $2,078,226. (2) Further authorize to extend three full-time temporary positions; two Social and Community Service Managers-4, Housing Continuum Administrator and Quality Assurance Coordinator, and one Miscellaneous Community and Social Service Specialists-7, Person-Centered Planning Liaison. Effective retroactive to July 1, 2025, upon G&C approval through June 30, 2027. 100% Federal Funds. Fiscal Committee approved on September 5, 2025.
