Authorize to enter into sole source amendments to existing agreements with the Contractors as detailed in the letter dated February 9, 2026, to continue providing community health care coordination for children with special health care needs, by exercising contract renewal options by increasing the total price limitation by $548,695 from $681,041 to $1,229,736, inclusive of a shared price limitation of $240,000 across the Contractors for environmental modifications and health-related services, and extending the completion dates from June 30, 2026 to June 30, 2027.
