Authorize to amend an existing retroactive sole source contract with The Mental Health Center for Southern NH d/b/a CLM Center for Life Management, Derry, NH (originally approved by G&C on 6-19-19, item #46), for the ongoing provision of Permanent Housing and Supportive Services to individuals and families who are experiencing homelessness through the Federal Continuum of Care Program, by exercising a renewal option, by increasing the price limitation by $403,069 from $576,904 to $979,973 and by extending the completion date from June 30, 2021 to July 31, 2021. 100% Federal Funds.
