Authorize to enter into a retroactive contract with Harbor Homes Inc., d/b/a Harbor Care, Nashua, NH, for case management and support services to individuals living with Human Immunodeficiency Virus, in the amount of $625,000. Effective October 1, 2020 through September 30, 2021, with the option to renew for up to two additional years. 100% Other Funds (Pharmaceutical Rebates).
