Authorize to retroactively amend an existing sole source agreement with Community Health Access Network, Newmarket, NH (originally approved by G&C on 9-20-18, item #13), to continue coordination and implementation of asthma health systems interventions to improve management of asthma, by increasing the price by $125,400 from $259,398.10 to $384,798.10 and by extending the completion date from August 31, 2020 to August 31, 2021. 100% Federal Funds.
