Authorize to amend an existing sole source agreement with Lamprey Health Care Inc., Newmarket, NH (originally approved by G&C on 3-8-17, item #9), for the provision of health promotion programs that promote health and independence opportunities to NH citizens, by increasing the price by $133,948 from $194,901 to $328,849, and by extending the completion date from September 30, 2020 to September 30, 2022. 85% Federal, 15% General Funds.
