Authorize to amend an existing contract with NH Coalition for Citizens with Disabilities Inc., d/b/a Parent Information Center, Concord, NH (originally approved by G&C on 12-20-17, item #22), to provide trainings and sponsorships to Family Support Coordinators who provide support to families with children with special health care needs through this contract with the Family-to-Family Health Information Services, by increasing the price by $15,000 from $715,366 to $730,366 and modify the scope with no change to the completion date of June 30, 2021. 100% Federal Funds.
