Authorize to enter into a sole source amendment to an agreement with Scott + Partners Inc., Essex Junction, VT (originally approved by G&C on 5-2-18, item #109), for Professional Services for the Glencliff Dementia Treatment Improvement, Glencliff, NH, by increasing the amount by $100,000 from $750,000 to an amount not to exceed $850,000; and by removing Phase III from the scope of work. Effective upon G&C approval through July 31, 2020. 100% General-Capital Funds.
