Authorize to enter into a sole source amendment to an existing contract with JSI Research and Training Institute Inc., d/b/a Community Health Institute, Bow, NH (originally approved by G&C on 6-21-17, item #51), to continue providing a broad range of support services across a number of public health programs, by increasing the price by $132,000 from $4,336,959 to $4,468,959 with no change to the completion date of June 30, 2021. 49% Federal, 51% Other Funds (Governor Commission Funds).
