Description

Authorize a renewal option to an existing contract with Classic Optical Laboratories Inc., Youngstown, OH (originally approved by G&C on 6-20-18, item #18), to continue providing vision care products and related services for eligible children and adults through enrolled Medicaid Vision Care Providers, by increasing the price by $160,000 from $316,000 to $476,000 and by extending the completion date from June 30, 2020 to June 30, 2022. 80% Federal, 13% General, 7% Other Agency Funds.
