Authorize to enter into a sole source amendment to an existing contract with NFI North Inc., Contoocook, NH (originally approved by G&C on 6-21-17, item #39B), for the provision of Care Management Entity services for the FAST Forward program, by increasing the price by $1,515,000 from $937,160 to $2,452,160, and by extending the completion date from June 30, 2020 to June 30, 2021. 100% General Funds.
