Authorize to amend an existing sole source contract with Mr. David S. Prescott, LICSW, Falmouth, ME (originally approved by G&C on 8-8-18, item #14), to provide independent risk assessment services to clients admitted to NH Hospital, by exercising a contract renewal option, by increasing the price by $18,000 from $18,000 to $36,000 and by extending the completion date from June 30, 2020 to June 30, 2022. 100% General Funds.
