Description

Authorize to amend an existing sole source amendment to an existing contract with Stephen F. Caldwell, D.D.S, Barnstead, NH (originally approved by G&C on 6-20-18, item #16), for dental consultative services, by exercising a renewal option by increasing the price by $158,080 from $149,760 to $307,840, and by extending the completion date from June 30, 2020 to June 30, 2022. 50% Federal, 50% General Funds.
