Description

Authorize to enter into a sole source agreement with Mascoma Community Health Center, Canaan, NH, to provide oral health care coordination services to assure ongoing access to preventive care, in an amount not to exceed $228,200. Effective upon G&C approval through August 31, 2022, with the option to renew for up to 2 additional years. 100% Federal Funds.
