Authorize to amend an existing sole source agreement with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 12-20-17, item #20), to continue assisting Critical Access Hospitals to improve quality of care for Medicare beneficiaries and to implement quality improvement activities in 13 Critical Access Hospitals, by increasing the price by $25,500 from $481,446 to $506,946 with no change to the completion date of August 31, 2021. 100% Federal Funds.
