Authorize to retroactively amend an existing sole source agreement with Community Health Access Network, Newmarket, NH (originally approved by G&C on 9-20-18, item #13), to coordinate and implement asthma health systems interventions to improve management of asthma, by increasing the price by $119,400 from $139,998.10 to $259,398.10, and by extending the completion date from August 31, 2019 to August 31, 2020. 100% Federal Funds.
