Description

Authorize to retroactively exercise a renewal option and amend an existing agreement with HealthFirst Family Care Center Inc., Franklin, NH (originally approved by G&C on 5-2-18, item #21), to provide services designed to improve breast and cervical screening rates in Belknap county, by increasing the price by $22,000 from $460,549 to $482,549, and by extending the completion date from June 30, 2019 to June 30, 2021. 100% Federal Funds.
