Authorize to exercise a renewal option and amend an existing agreement with the Area Agency of Greater Nashua Inc., d/b/a Gateways Community Services, Nashua, NH (originally approved by G&C on 2-1-17, item #20A), to continue the provision of Financial Management Services, by managing payments to family caregivers for respite and support services they receive, by increasing the price by $867,355.55 from $2,098,881.10 to $2,966,236.65, and by extending the completion date from June 30, 2019 to June 30, 2020. 50% Federal, 50% General Funds.
