Authorize to exercise a renewal option to an existing contract with NH Coalition for Citizens with Disabilities Inc., d/b/a Parent Information Center, Concord, NH (originally approved by G&C on 12-20-17, item #22), to provide Family-to-Family Health Information Services that support families as caregivers for children with special health care needs, by increasing the price by $357,950 from $357,416 to $715,366, and by extending the completion date from June 30, 2019 to June 30, 2021. 25% Federal, 75% General Funds.
