Authorize to exercise a renewal option and amend existing sole source contracts with the vendors as detailed in letter dated May 8, 2019, to continue providing person-centered Healthcare Homes that deliver integrated primary care, community behavioral health care and wellness services for young people with serious mental illness and serious emotional disturbance, by increasing the price by $3,509,271 from $1,448,017 to an amount not to exceed $4,957,342, and by extending the completion dates from June 30, 2019 to June 30, 2021. 100% Federal Funds.
