Description
Authorize to exercise a renewal option and amend an existing sole source contract with Dr. David A. Bogacz, Concord, NH (originally approved by G&C on 4-6-16, item #10), to continue to provide dental services to the residents at Glencliff Home, by increasing the price by $40,000 from $57,000 to an amount not to exceed $97,600, and by extending the completion date from June 30, 2019 to June 30, 2021. 20% General, 80% Other Funds (Provider Fees).
