Authorize to exercise a renewal option and amend an existing sole source contract with Healthdirect Institutional Pharmacy Services Inc., Concord, NH (originally approved by G&C on 7-21-17, item #35), to provide pharmaceutical services to the residents at Glencliff Home, by increasing the price by $34,500 from $34,500 to an amount not to exceed $69,000, and by extending the completion date from June 30, 2019 to June 30, 2021. 80% Agency, 20% General Funds.
