Description

Authorize to exercise a renewal option and amend an existing agreement with National Jewish Health, Denver, CO (originally approved by G&C on 6-24-15, item #51), for the provision of statewide telephonic tobacco treatment services in order to reduce the health and economic consequences of tobacco use and dependence, by increasing the price by $405,000 from $1,496,235 to $1,901,235, and by extending the completion date from June 30, 2019 to June 30, 2020. 31% General, 69% Federal Funds.
