Authorize to enter into a sole source amendment with Granite Pathways Inc., Concord, NH (originally approved by G&C on 10-3-18, Late Item A), to enable the contractor to access State Opioid Response grant funds for the provision of residential adolescent Opioid Use Disorder treatment services, by increasing the price by $90,000 from $0 to an amount not to exceed $90,000. Effective retroactive to January 1, 2019 through September 30, 2020. The underlying contract completion date for services to be provided remains unchanged as October 31, 2022. 100% Federal Funds.
