Authorize to enter into a retroactive sole source amendment to an existing contract with the Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 9-27-17, item #13), for the provision of assistance and support for the 13 NH small rural hospitals in order to implement activities provided annually by the Small Rural Hospital Improvement Program Grant by increasing the price by $77,357 from $493,691 to $571,048. Effective April 1, 2019 through the original end date of May 31, 2021. 100% Federal Funds.
