Authorize to amend an existing agreement with Mary Jane Sullivan, Hampton, NH (originally approved by G&C on 3-8-17, item #16), to provide Early Hearing Audiologist Consultant Services, by increasing the price by $28,000 from $43,819 to an amount not to exceed $71,819, and by extending the completion date from June 30, 2019 to June 30, 2021. 100% Federal Funds.
