Authorize to exercise a renewal option with a sole source amendment to an existing agreement with one of the contractors as detailed in letter dated November 14, 2018 (originally approved by G&C on 5-2-18, item #20), for the provision of Targeted Prevention Programming for DCYF-Involved Families with Substance Use Disorder, by increasing the price by $1,166,506 from $744,198 in the aggregate to $1,910,704 in the aggregate, and extending the completion date from June 30, 2019 to September 30, 2020. 100% Federal Funds.
