Authorize to enter into retroactive sole source agreements with the vendors as detailed in letter dated October 25, 2018, to implement person-centered Healthcare Homes that will ensure delivery of integrated primary care, community behavioral health care and wellness services for young people with serious mental illness and serious emotional disturbance, in an amount not to exceed $1,039,880. Effective October 1, 2018 through June 30, 2019. 100% Federal Funds.
