Description

Authorize to enter into a retroactive sole source agreement with the Community Partners, Dover, NH, to implement person-centered Healthcare Homes that will ensure delivery of integrated primary care, community behavioral health care and wellness services for young people with serious mental illness and serious emotional disturbance, in an amount not to exceed $408,191. Effective October 1, 2018 through June 30, 2019. 100% Federal Funds.
