Description
Authorize the Division of Public Health Services to amend an existing agreement with Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 12-20-17, item #20), to continue assisting Critical Access Hospitals to improve quality of care for Medicare beneficiaries and to implement quality improvement activities in thirteen Critical Access Hospitals, by increasing the price by $27,300 from $454,146 to an amount not to exceed $481,446, with no change to the contract completion date of August 30, 2021.
