Description

Authorize to retroactively exercise a renewal option and amend an existing sole source contract with the City of Nashua Health Department, Nashua, NH (originally approved by G&C on 12-7-16, item #12A), to continue providing Healthy Homes and Lead Poisoning Prevention care coordination services, by increasing the price by $50,000 from $50,000 to $100,000 and by extending the completion date from September 30, 2018 to September 30, 2020. 100% Other Funds.
